504 for eosinophilic esophagitis

I have a student, not new to the district, not new to the diagnosis, that we are meeting on soon for a 504. Do any of you have students on a 504 for this? What do you include? I'm trying to cover all aspects but we have had no issues with this particular student where this dx is concerned so I'm afraid I'm not thinking of all that might be need to be covered. I'm actually uncertain as to when a 504 plan seemed to be warranted.

I used this site when a parent was uncertain if we needed a 504 for her son. Lots of good stuff and written from a parent's perspective. It might give you a starting point.
