Hi, everyone, I'm confused too...I saw it on the website it says you don't need to fill out the verification form 3 if you are using nursys. And on the bottom of the website it says, if you hold any other professional licenses in states other than New York, you must also use Form 3 to verify that licensure to New York State...?? Did you send the verification form too or not? Can someone helps us out? If I really need to send the verification form, where did i find the information on how to send it to the NJ BON? Thank you.

I,too, am trying to work in New York from another state and I just received my license in the mail a week ago!
I only Mailed Form I, Form II, Child Aubuse Training result, and Infection control results. I sent Form 2 to my school. I did not send Form 3...I did www.nursys.com to verify my license.
I'm looking back at the website and I see what you guys are talking about with the conflicting information--completely missed that disclaimer!
So, in short, no I did not send Form 3. I did verification online. It took them 2 months to give me my license (october-december). I called them after 1 1/2 months and they were very nice in telling me the progress of my application.
Sorry I can't be of anymore help but Good Luck!!
NONE OF IT MAKES SENSE. To verify NY license to lets say Vermont it costs 20 dollars in paper form. To verify same information through NURSYS it costs 30 dollars. So NY has the form but is part of NURSYS so you are forced to pay the 30 dollars. It is all about money. CRIMINALS>
Hi everyone,
I have a quick question. I am currently licensed in New Jersey, and I am endorsing my license to New York, and there is a specific form "form 3: VERIFICATION OF OTHER PROFESSIONAL LICENSURE/CERTIFICATION"
Link: http://www.op.nysed.gov/prof/nurse/nurse3.pdf
Does this form STILL have to be filled out and sent if I apply for verification Nursys.com?
It states: If you are licensed as a nurse in another state, you must provide the New York State Education Department with verification of that licensure. The National Council of State Boards of Nursing (NCSBN) handles verification of licensure for a majority of states through their Nurse System (Nursys). You can check to see if the state(s) where you are licensed as a nurse participates in Nursys by visiting their Web site at www.nursys.com
or by calling them at 1-866-819-1700. If the state(s) where you are licensed as a nurse participates in Nursys, you must request verification of your licensure from Nursys, not the state(s). If your state(s) of licensure does (do) not appear on the Nursys list, you must use the Verification of Other Professional Licensure/Certification form (Form 3) to verify your licensure to New York State.
Please note, if you hold any other professional licenses in states other than New York, you must also use Form 3 to verify that licensure to New York State.
Sorry, I am just confused, I know that New Jersey is eligible for verification at nursys, but am wondering if I still have to send out Form 3 if I am using nursys for verification. Hope this makes sense..thank you!
the problem is that you are forced to pay 30dollars instead of 20dollars. here we go: NUYSYS verifies license in another state. Thats great that it takes 10 seconds or even a week. But your application to the other state is going to take longer. so the purpose is to take 10 dollars from everyone or steal it I should say. This should not be allowed especially if other states paper-form is less than 30 dollars.
