Employee Authorization To Release Personnel Information

Sorry if this is not the right thread, I'm new to the site. I've never filled out that form before. I'm not sure what information i should consent to releasing. So I did my research online but couldn't find any information about the pros and cons about letting the HR release certain information about you.

Here's the personnel informations they're asking for consent on the form

Salary Rate (per annum) Home Address

Length of Employment Social Security Number

Date of Termination Employment Status with hospital (Full-time, Part-time, etc.)

Reason for Termination

I don't see anything wrong giving the HR the consent to release my salary rate, length of employment, date of employment and employment status with hospital. And Social Security Number and Home Address are basic information requirements. Date of Termination and Reason for Termination are out of the question. So if anyone could give any pros and cons about this document would be much appreciated

"Termination" isn't necessarily a negative word. It simply refers to an ending of some sort. When you quit a job, you're terminating your employment with that employer. In that case the HR information would read something like this: Date of Termination: 10/20/2013 Reason for Termination: Resignation. If a person were laid off due to budget cuts, the reason for termination would be "Laid off" or "Services no longer required". Many people see "termination" and immediately think "FIRED" but that's often not the case.

When a prospective employer is screening applicants using a standardized form and certain areas - such as these ones - are left blank, it can raise red flags, as if the applicant is concealing something. Ultimately, you will have to decide for yourself what you're comfortable with having released. Just be aware of the implications.

Just to be clear - is this a form that your current employer is asking you to sign as part of your resignation/termination? If so, I think it is OK. That just means that they can provide more than just 'dates of employment' and 'eligibility for rehire' to prospective employers. If everything is kosher, there shouldn't be a problem.
