Are there any schools that offer an associate's in nursing 100% online? Meaning, is it possible to take the courses online and take the clinicals in a hospital or university in a completely different city/state?

There are going to be no completely online with clinicals in random places prelicensure programs. There is Excelsior College that does work with students who have previous healthcare experience, such as LPN, paramedics, and certain military corpsmen. The reason being that hospitals develop relationships with nursing schools, and there are many nursing schools that are competing for clinical placements. Prelicensure education really needs to have that hands on component. The other issue is that there are some states that will not license Excelsior grads. So, if you do find a school that provides what you want, find out if it is accredited by either ACEN or CCNE, and whether it will permit you to become licensed in the state you wish to work in.

ISU has an LPN to BSN accepted in a few states where coursework is online/distance and you must work with the school to set up local clinical preceptorships. (Medics and RRT are ineligible only current LPNs)
Excelsior offers distance based classes and challenge exams for currently licensed LPN's and medics (RRTs are no longer accepted) with a 3-day in person clinical challenge exam in NY, WI, GA, or TX at specific facilities.
Neither is accepted in CA.
There are no entry to practice programs that offer distance/online education.

Not for ADN, but there are for BSN: Accelerated Online Bachelor's to BSN — College of Nursing
Interesting an online ABSN program for those with a BA or BS in another discipline...
The have been around for a while - not a new program. I gather it is very competitive to get in!
