How do you communicate with the dialysis centers? What type of info do you give or get back when they come back?

hi there, do you work in a nursing home? i work in a dialysis unit and the pt's that are in nursing homes usually come with a communication sheet that we fill out and dens back with them. is this what u were talking about?

The diaylsis center that we work with has our units fax number. If we don't hear from them then fine, kind of like no news is good news. If they have concerns, questions, lab work or recommendations they fax us the info on one of their fax sheets.
They have also made it very clear to us that if we have question, they are more then willing to visit with us.

We send a binder back and forth. Any information to share from either facility goes in the binder. Anything REALLY important rates a phone call.
Yeah...I was just wondering what everyone else did. Sometimes we get a call, alot of time they tell the families about med suggestions/ changes but never call or write us a note. Sometimes it is a few days after the fact.
We used to do a binder, I'd like to get back to that.
