Diabetes Documentation

Hello everyone!

NYC school nurses are expected to use a log titled "Diabetes Documentation Form" but some nurses think it could use improvement, so I've been wondering what other nurses have been using. 

Do you use a specific log for tracking of all diabetes documentation/treatments? Do you find that it's useful? Is there anything you would change?

We use SNAP and their recommended templates for diabetes documentation. Their recommendations actually match the state's mandated reporting procedure, so when we submit our monthly/yearly reports, as long as we are using the correct templates, the information is automatically pooled for this report.

For routine BG checks, carb coverage, we work off a med/treatment schedule (essentially an eMAR) within the software. If a student's is experiencing symptoms, we have note templates for hyper/hypo glycemia, and chart within that, and use PRN meds/treatments if part of the order set. 

I do it all in my EMR (Schoolbrains, which I know like no one uses).

For a couple of my students, we also do a paper log of BG, carbs eaten, insulin given, ketones check (if needed). This is so they can take that simple paper log to their doctor's appointment.
