Hello, what happens if, in a bizarre moment of forgetfulness after 12 years of working with ports, you suddenly de-access the port WITHOUT flushing? The IV fluid that was running was clindamycin. After infusing the clindamycin, the port was de-accessed, no saline or heparin flush administered.. please help

It needs to be reaccessed and flushed asap

Oncology nurse here who deals with ports daily. You absolutely have to reaccess, flush and heplock. It happens! We are all human

Another oncology nurse here who agrees with the above. Port needs to be reaccessed and flushed. And yes, we all make omissions sometimes.
