  1. De-accessed Port w/o Flushing! Please help!

    Hello, what happens if, in a bizarre moment of forgetfulness after 12 years of working with ports, you suddenly de-access the port WITHOUT flushing? The IV fluid that was running was clindamycin. After infusing the clindamycin, the port was de-accessed, no saline or heparin flush administered.. please help
