I'm looking for a sample 5 day discharge notice. Anyone know of where I might find one? Thanks

Why would you need a 5 day discharge notice? I thought the reg was 48 hour notice.

Our's is 48 hrs also.

When I was laid off due to downsizing, I was told in person, by the DON (although the other nurse who was laid off called me on the phone and let me know what was happening). There was no written notice at all. At least none that was handed to me. When they asked me to come back, that was done over the phone. Also, I was told ten days prior to the effective date of the layoff. Someone told me that was necessary because of the law.
WE were, I think, talking about discharging PATIENTS not downsizing or laying off staff.
Well, there obviously was not enough info in any of the posts to make that clear to me, a reader. Pardon my inability to read minds. Besides, since when is there a discharge notice anyway? I've always just been discharged. The only discharge notice I've ever heard about is the three day notice mandated by law, when a home health care agency is discharging a client. I would suppose that any facility that intends to discharge a patient without their full cooperation might use some kind of legal document. Now that the REAL intent of the thread has been exposed. Thank you. Hope the OP does not come back and inform me that there was a third intent to the thread. But then, that might be informational.

Calioter, you probably didn't ever get a discharge notice because you probably don't have Medicare. The Medicare and Managed Care regs are very clear and state a facility must give the resident a 48 hour notice of discharge unless the discharge is an emergency such as a trip to the hospital with a subsequent admission to acute care.

I picked up a tone in your post and responded to that. Sorry if I misinterpreted your intent. I think it was your capitalization of the word WE, made my little brain think I was being excluded or stupid or something. Who knows? But I really did respond to the thread as I understood it. There are times when there can be misinterpretation of posts. Glad you talked about your interpretation of what the OP meant. What you said really is helpful to me as I read these posts for information as well as just having something to do! Have a great evening taking care of those residents of yours!!

This is correct. We have to give all Medicare patients a 48hr DC notice if patient is remaining in the community with the exception that the patient is requesting discharge from HomeCare. In this case you don't have to give one.
