Does anyone do this? I haven't been and am wondering if maybe this is why I'm not getting interviews (well, I've had two but neither were LTC). I've just been attaching my resume to the application.

I've never attached a cover letter to a nursing home employment application. I just submit the application in person, and often receive an interview on the spot. If the hiring manager wants further information in the form of a cover letter or resume, then I'll submit those.
However, you're in a competitive geographic region where nursing jobs are sporifice, so perhaps a good cover letter would help you stand out positively.
