I took a course that I paid a fair amount of money for and at the end of the course, I received a work call and stepped out. I guess I was distracted and did not pick up my CEU certificate. I need this because I am applying for an additional license and need to list all my CEUS. I realized I was missing this one so when I emailed to get this, I was told that I must not have filled out an evaluation form and that was it. I just need a copy for my records so I have it and to know how many ceus the course was. so now I feel that my ceu's are contingent on an evaluation form? I realize that they would like the evaluation form but this seems unfair since I did pay for the course. Can they refuse to give me my CEUs if I did not fill out an evaluation? I thought I did but just never picked up the certificate. If they are anonymous anyway, how would they know for sure.

Ask them if/how you can complete the requirements for the course now. It might be possible: it might not be. That might not seem fair to you, but look at it from the other side of the fence. In order to qualify as CE, they need a mechanism to ensure that people actually stayed for the whole course and didn't just pay their money and skip the course. The credit is given for completing the course, not for just paying the money. So I assumed that everyone signed in at the beginning ... then they had the evaluation form to hand in at the end to verify that the person stayed for the entire event.
Do you have any witnesses that you stayed for the entire program? That might help them feel comfortable with giving you a chance to complete the evaluation form now ... or do some other activity to establish that you stayed for the entire course and didn't skip out early.
Good luck. The rules for giving CE credit are pretty strict. If you didn't fulfill the requirements for credit, you might not be able to get it after the fact. But humility and witnesses might help.
I can repeat the skills I learned without a doubt. The instructor might remember me but I wouldnt even know how to go about getting him to remember if I stayed the whole time, etc., send him my picture, explain where I sat in the class. I dont know anyone in the class, I took it at a university that is several hours away from where I live. It was a fantastic class and I waited over a year for it so there is no way that I was leaving early. I am angry that I paid for this class and now am having a hard time getting the certificate. And the explanation you are giving at least makes sense, I was not offered an explanation, just an email that said I must not have handed in the evaluation. Period.
