Alot of the paper work, from reading drs orders to transcribing admitting and transfering drs orders to nurses notes. please help me
I have been through orientation, and am still lost in this area. Is there a link or site you can offer me that will help me better understand this pertinent part of being a successful LPN? Also can you provide me with your experiences with this that will make it easier. I certainly would appreciate it

Best to take time to look at examples at your workplace. Have a talk with your supervisor and ask for extra time to do this. If you can come in on your own time and go in a corner where you are not in the way of others that would be good. Make yourself an 'example' binder if you like (of course, make it HIPAA compliant, patient John Doe or no name at all).

Do you have a nurse preceptor or clinical nurse education department ? Did they review documentation during orientation? Ask your preceptor, nurse educator, manager for assistance to ensure your documentation meets facility policy & procedure.
