confuse about"notify the physician"

the kaplan book says don't choose the answer that"notify the physician".this is the exam to test you---nurse,not the doctor.

ok,i always rule out the option with"notify the physician". but,in the kaplan test trainer, Its not work .the correct answer always:"notify the physician".

I really confuse! what should I do for critical thinking? plz help. thanks!!!!!!!!!!!!!!!!!!!!!

If notify the physician is the ONLY answer that mentions the physician and the other 3 options are nursing implementation then choose the physician.

It all depend. If you're an LPN you need to notify your charge nurse but if is an RN you notify the physician in NCLEX world but in in real setting.
