Was this in Virginia? It is in the contract that you agree to do all the minutia paperwork and one time however she def would have been given warnings. My employer often was late and they would email me 2 or more times before threatening any discipline. Likely if they said she was dismissed she can request a meeting with the case manager and or the board and sign a new contract and be re accepted into the program. I've known many people be dismissed and return for much worse.  Just needs to talk to her case manager.

Was this her self report form or one from her employeer/Dr/etc? My case manger was very understanding if my counselor was late turning in her monthly paperwork because that was largely out of my control but I think he wouldve killed me if I was late with my monthly paperwork LOL I am not in VA but in my state, the paperwork isn't even due until the 10th of the month, so I would always just turn it in on the 1st. 

It is in our contract that we submit forms monthly by the 10th. They can absolutely kick you out of the program for not abiding by the contract. How many times was she late?? The forms are super annoying to fill out, but only take me 15 minutes or less to complete and they are required. They are very diligent with warning letters for these types of things. I can't imagine her being kicked out if she was putting in effort and communicating? But I have seen some crazy things happen. Hopefully she can take the necessary steps to get back in the program. 

HPMP keeps denying jobs I am offered and it's starting to feel impossible. I've tried places with no narcotics, was told not enough supervision by worksite monitors, dialysis was too many hours (45 instead of 40) multiple locations. Can anyone recommend a job in northern VA area that they know of? I thought dialysis was a safe bet but I am starting to feel pretty defeated. Thanks so much.

They are very, very specific about the requirements. It must be exactly as they say, and your monitor must agree to be able to do as they request. I lived in NoVA. I did a call center triage and they approved that. I know two other girls who also did similar call center situations. There was another nurse who did a pre-op/intake type situation and also no narcotics.  I do know many that also did dialysis but it must have been the hours. As much as it seems tedious, they truly are trying to set you up for success, and fewer hours to start is less stressful. Maybe revisit the dialysis and see if the hours can be adjusted or look for another location with fewer hours.  Hang in there. I know right now it seems like no end in sight, but it does get better. I promise.
