chronic cpap

This may be a dumb question, but how long will a nicu keep a baby on cpap before deciding that the kid needs to be trached? I have a kiddie who has been on cpap of 8 in a significant amount of 02 (usually around 40%) and has failed attempts to wean the pressure. This kiddie has been on cpap almost 2 months. Are there any other options? THANKS

We get chronics on CPAP for ages and ages, even with diuretics etc. I presume this is because we use CMV (PCV) on every baby that won't breathe....so we basically screw up their lungs for them in the first week of life. We only use HFOV as a rescue method if we REALLY can't oxygenate, diaphramatics and hypoplastic lungs etc. I've seen
