Chiari malformation care plan

Does anyone hve a care plan for Chiari malformtion? I have looked and have not found one that applies to school.

With stuff like this, and especially something where there are varying degrees of a condition, I usually just refer to the MD orders as the care plan - if there are restrictions, precautions, interventions that may be needed at school, classroom modifications, medication, etc.

I did this as well when I had a student with this. She just had precautions and an OTC med order prn.

I had a parent who made her own care plan. It explained her condition and when to call mom. She met with all the staff together who were involved with her care and gave everyone a copy.
