Ccm entrant needs help with requirements

Hello, I am an RN in california and am planning to take CCM examination under the 2 year requirement. I have been doing self study and am only worried on acceptable job description with all the key points covered. I would like to ask if anyone can post a sample that worked with all of the points covered. I need it to cross reference what I currently have as I am not sure of the current wording. Also if I get denied but know that a poorly written description is at fault what steps can I take to qualify me to sit for the exam with the experience I already have. Anyone that has been down this path before I am just pre planning for all outcomes, intend to sit for August. Thank you.

I don't really understand what your question is but to make it easy for you and me to answer, I will tell you what I did to apply and pass my CCM. I have been a HH RN for several years, case management in home health is the daily work. When I completed my application I simply entered what I do on a daily basis, including the amount of hours, my education (RN Degree is enough) and my supervisor's names and emails (never asked if they were contacted or not) then my application was approved.

now, related to the test. I bought a course from PRIME Case management that was not as concise as I expected but gave very good information and Q&A (you can buy the Q&A separated without paying for the whole program) then I bought access to previous Q&A from the commission and answer those several times, then took the exam. The exam is not a hard test, it will only asks what you do on a daily basis but very simple (remember a standardized test only measures minimal knowledge) and some questions even if you don't know you will be able to guess using common sense. Um, there's no such thing as an "RN degree." The CCM requires a BSN to sit for the exam, or other 4 year degree in a human service focus. If you are an ADN RN you cannot sit for the exam.

The CCMC *will* verify your experience down to the day, by contacting the supervisors listed and HR departments if necessary. They review job descriptions and hours worked carefully.

I'm not sure what the OP is asking either, the post is so poorly worded and written I have no idea what exactly it is they're asking about.

SECTION 4:

qualifications:

To be eligible for the CCM Exam, you must hold a:

1. Current, active, and unrestricted licensure or certification in a health or human services

discipline that within its scope of practice allows the professional to conduct an assessment

independently.

OR

2. Baccalaureate or graduate degree in social work, nursing, or another health or human services

field that promotes the physical, psychosocial, and/or vocational well-being of the persons being

served.

The degree must be from an institution that is fully accredited by a nationally recognized

educational accreditation organization, and the individual must have completed a supervised field

experience in case management, health, or behavioral health as part of the degree

Ok, thank you all for the responses. I've since applied and been accepted "immediately" and am just waiting for testing. The original post is simply asking for a job description, as in your case management position, they need to job description that qualifies as a case manager. I am confused why that is not understandable but thank you all^^
