case about obstetrics(i need your help)

your sleep is inturrupted by the tones of radio and the dispatcher saying:bravo 3, stand-by for call"you swing your feet into the floor and grap the radio"go ahead, dispatch,"you reply

bravo 3, respond to 77 cherry tree lane for pregnant pt in labor.the other dispatcher is still on the phone with the husband.Additional information to follow.

you and your partener head to the ambulance and start towerd the scene. About 3 minutes from the patients house,the dispatcher tells you that baby appears to be crowning and the husband is still on phone getting instruction. You and your partener decide you need plan. you agree that both the adult and pediatric equipment need to be brought in, as well as the OB kit.

you also agree that you will focus on the baby after delivery and your partener will provide care to the mother.

Questions

1-what should be the first priority when entering the scene?

2-should ALS assistance be requested?

3-What questions should you ask mother or father?

I am curious.....Do emergency nurses in your country get dispatched to homes?

Think about it......you are entering the room, your focus is the baby..... what would concern you first?

Is the baby born?

Is it breathing?

Is the cord still attached?

Is the baby active?

Is the baby crying?

Is there by distress?

Was the fluid colored?

What is the cap refill?

Is childbirth in the BLS scope of practice?

What would your priorities be on a call involving bleeding, possible airway obstruct?

What info do you think would be pertinent? Would you need to know things like how far along is mom? Does mom have any medical history? Just the answer to those questions could change the answer to the first two questions you asked about the scenario.

Always, always, ALWAYS start with your basics ABC's.

What does your text book say?

What would you want to know bout the Mom...what about the fluid? How many pregnancies?...how many live births? Whenare you due? and How are they having twins? would be important to know....then what?
