I'm researching my careplan so I can care for my patient tomorrow...on the nursing rounds report (which we bring home) it says DX is....

N/D.....what is that?

I can do renal failure. THAT one I know!

Yikes!! I just looked it up in my medical abbreviations book...I think we can safely eliminate "natural death":uhoh3:

There's nasal deformity, nasoduodenal, neck dissection, neonatal death, neurological development, neurotic depression, Newcastle disease, no data, no disease, nondisabling, nondistended, none detectable, normal delivery, normal development, nose drops, not detected, not diagnosed, not done, nothing done, and nodular and diffuse.

I'd think it likely means Nausea/Diarrhea. Without having additional info like what kind of unit or pt hx, it's hard to say for certain. But I'd be willing to risk a $1 that it's GI.
