help i need two interventions for my patient with malignant stomach ca..n also a nursing diagnosis..He goes to radiation therapy..hes 78 male..

How 'bout Altered Nutrition: less then body requirements, r/t pain, nausea...sounds like a patient I had in school.!
etiology???interventions?patient outcome n rational from diagnosis???
patient is on gtube feeding..isosource

Try dly wt, same time dly, to keep an eye on any wt loss. Hope this helps.
Add: decreased absorption of nutrients associated w/ impaired digestion resulting from rapid entry of food into small intestin (result of reduced stomach size/removal of pylorus)
Assess for and report s/s post vagotomy diarrhea (watery stools, urgency, abd cramping) 1.) advance diet (or increase feeding) gradually
2.) administer anti diahrheals if ordered.
3.) consult MD if persists

Anxiety related to radiation therapy and insufficient knowledge of treatment and self-care measures
Interventions: Encourage patient to express his feelings about his condition, progress, prognosis, the effects of his treatment on his life style and treatment
Assess for signs of a negative response to changes in life style such as refusing to discuss life style changes, denial, gradual refusal to perform self-care, social isolation
Assist with hygiene and grooming as needed
Use energy conservation techniques
Monitor for diarrhea, abdominal pain, skin changes, weight loss, dyspnea, cough, chest pain
