Job Description: Continuous Care nursing: caring for patients at very end of life or during crisis (usually pain medication evaluation/adjustment) working 12-hour shifts in patient's home, nursing home, assisted living facility, or hospital. Visit Nurse: making home visits to assess patient needs and/or administer treatments; educating primary caregiver/family. Inpatient Care Unit: caring for inpatients in hospice facility.
