Hey. I am a first semester nursing student. It is taking me hours to do my assessment tool, pathophysiology sheet and care plan-- Any recommendations on good care plan books would be much appreciated as well as helpful tips. I work full-time, have children, husband, Sunday school as well as studying for tests and lots of reading for my classes. :uhoh3: Thanks for any and all help.

Oh My!!! Your plate is full!!! I use the book that was required of us to get for school. It is the best I have found. It has the nursing diagnosis, the related to part, the definition, assessment, defining characteristics, associated medical diagnoses, expected outcomes, interventions and rationales,evaluations for expected outcomes, and needed documentation.

It takes 10 to 15 minutes to complete one!!! I got all 100's on all my careplans that I did.

The book is called NURSING DIAGNOSIS REFERENCE MANUAL

I have had the pleasure of redoing nurse's notes two weeks in a row now--I am doing okay in everything but this and this is getting on my last nerve. My instructor is busy when we go for clinicals, I go in, meet my patient, I am told VS are at such and such time, we give meds at such and such time, I start assessing what all I need to (I know I am not near where I need to be but so far my assessment tools are good, I am getting the info I need--I am just stinking at remembering times and knowing what and what not to put in the notes and my instructor is busy, busy, busy and I am feeling positvely mentally challenged at this moment --cajunnga

I like the Sparks/Taylor book, too. I used it until I got care plan stuff on my Palm Pilot. The book is very easy to look things up quickly, and to cross-reference.
