Care Plans-Help...Please

:uhoh3: Hi everyone! I am starting my second semester in clinicals and I am lost! We have 2 do care plans, teaching plans and document in APA. I don't know where to begin. Does anyone have any ideas or suggestions?

Maybe invest in a good care plan book?

I have Sparks & Taylor Nursing Diagnosis Reference Manual, as well as a Maternal-Newborn Nursing Care Plans book.

Buy a book that has APA format and buy a care plan book. That thing saved my life last semester, and our instructor actually told us to buy one, even though it wasn't a required text. That came in just as handy as my medical dictionary my first semester! :) Just personalize it to your patient's needs. I have Saunders All-in-One Care Plan Book. Good luck! :p

Your textbook probably has all the info you need, just not outlined clearly. If you learn to find the info in your textbook you will be able to locate it EVERY time you need it. But maybe these sites can get you started.

The books and the sites that have been posted are great and should help you. I posted a careplan template that my clinical instructor gave us that has helped me A LOT, I'll look for it and bump it up for you.

I have an awesome care plan guide that is divided into chapters on the disease, rather than nursing diagnosis. So if your pt is in chf, there's like 8 different nursing diagnoses, careplans... that include rationales. It's called "Nursing Care Planning Guides" by Ulrich and Canale. It's blue. It's been such a great tool.
