This website is highly recommended by all my friends if you have problems writing care plans. It is actually a care plan generator, all you do is pick what applies to your patient and out pops a complete care plan. I would like to stress DONT RELY to heavily on this website because in my own personal exp. you may have to write a care plan whenever and wherever you instructor see's fit. Make sure you know how to become a generator yourself before using this. :)

If you need a nursing journal article, heres your place. It cost money for most of the articles but some are free. In my exp using it I was always able to find a free article for just about any topic I needed. Make sure if it says it cost money for a article to click on it and scroll down, because sometimes it only cost money if you print the article. Great for Nursing research papers. :)
here are links to the 75 nursing diagnoses that are currently in the elsevier care plan constructor companion website for nursing diagnosis handbook: a guide to planning care, 7th edition, by betty j. ackley and gail b. ladwig. each of these pages contains the nanda definition, outcomes, nursing interventions and references. there are currently 188 nanda-i officially approved nursing diagnoses (per nanda-i nursing diagnoses: definitions & classification 2007-2008 published by nanda international, page ix), so this list is hardly inclusive. however, it does contain the nursing diagnoses most commonly used in the medical/surgical areas of the acute hospitals and in nursing homes.

the care plan book that I use is sparks and taylors nursing diagnosis reference manual ISBN: 1-58255-292-4 if have found that it is the best for me. we dont use care plans at my school we use the concept mapping technique. check this book out it is really good.

there is also a book that has been published called concept mapping: a critical-thinking approach to care planning by pamela mchugh schuster that costs about $26 and has about 173 pages. this author is an instructor in a nursing school that uses concept mapping. she goes through the steps of the nursing process and how to apply concept mapping with the steps of the nursing process. there are a number of forms scattered throughout the book. it is written and aimed at student nurses. it has a separate chapter on concept mapping patients with psychosocial problems. it also includes two appendices which have the current nanda nursing diagnoses classified and sorted by (1) maslow's hierarchy of needs and (2) gordon's functional health patterns.
