Care Plan!!!!

[color=#333333]i need help in writing my care plan i need goals outcome criteria interventions, rational and evaluations for the nursing diagnosis imbalanced nutrition: less than body requirements r/t poor infant feeding behaviors. can anyone help me? thanks so much!

    infant will feed for at least 20 mins at every feeding

    infant will not lose more than 5% of birth weight

    [h=6]outcome criteria[/h]infant will engage mom's entire areola by 4/10/12

    infant will breastfeed for 25 minutes by 4/10/12

    infant will gain weight by 4/10/12

    interventions rationale and evaluations i have yet to complete

    Evaluation is evaluating your goals -- whether or not they were met. So if you're writing the plan now, you won't have the evaluation yet. Rationale should be easy-ish. Just explain why each of your interventions supports your goal.
