These goals vary based on the patient.

A short term goal would be something like:

By the end of the clinical day (measurable), after teaching session, will demonstrate how to draw her own insulin.

By discharge date (measurable), patient will verbalize the s/sx of hypo/hyperglycemia, what to do on a "sick day", demonstrate the use of a blood glucose meter, demonstrate the use of a BS/insulin diary and verbalize her new healthy eating & exercise plan

Long term goal

Within 1 month, the patient will have consistent blood sugar control (between 90-120 mg/dl)

So these should be correct?

Client will:

• Verbalize understanding of individual risk factors that contribute to possibility of falls and take steps to correct situation by 11/16/08

• Demonstrate behaviors to reduce risk factors and protect self from injury by 11/16/08

• Demonstrate lifestyle changes to reduce risk factors and protect self from injury by 12/16/08

• Be free of injury until discharge

• Follow medication regimen until 12/16/08

NOT "Safeguard from falls/associated injuries"

based on what you've posted i am assuming the diagnosis is risk for falls. the overall outcome for this diagnosis in its broadest sense is for the patient not to have any falls.

client will:

i don't think this is measurable enough. in your interventions, you would have included a number of risk factors around the patient that you discussed needed correction. i would identify one
specific
one that the patient will identify by 11/16/08 as a contributor to their risk for falling and have them state 2 things that they will do to correct the situation.

* demonstrate behaviors to reduce risk factors and protect self from injury by 11/16/08

"behaviors" is too broad a term and alludes specificity. by "demonstrate" you mean for the patient to show you things they can physically do to prevent themselves from injury, right? this means you had interventions showing them how to physically protect themselves if they fell? then, i would say, "return demonstrate 3 ways to physically protect themselves from injury of a fall by 11/16/08."

* demonstrate lifestyle changes to reduce risk factors and protect self from injury by 12/16/08

injury is really not a component of
risk for falls
since the focus is trying to prevent them. changing lifestyle activities that would lead to falls is ok, but you need to account for identifying what those lifestyle changes would be and how they are going to be modified. i don't think "
demonstrating changes"
is the correct wording to use here. interventions for this would likely include having the patient analyze their activities and consider their risk in relation to falling. then, they need to decided how they could best change or alter those activities while still doing them. the outcome would then be to "
report at least 3 lifestyle changes they have made which reduces risk factors and protects them from falling by 12/16/08.

what does following a medication regime have to do with
not
falling? was falling a direct side effect of not taking their medication?

I had a list of nursing diagnosis that I was using and so these are just a few goals pertaining to one or another nursing diagnoses, not just for risk for falls. I got these goals word for word from a nursing diagnoses handbook so that is what i turned in and it was fine for my instructor. Thank you for your help

you are kidding! i am not being mean, but shame on you. you don't learn anything by copying from a book. your goals should be an extension of your nursing interventions. if i were grading your care plan and your interventions didn't add up to what your goals were--ding!

i spent a good deal of time giving you some response to your post. i hope you learned something from it.
