Yup, us USians think you're talking about a package of goodies you send to your kids when they go away to school/camp/war, which is undeniably important, but a rather odd nursing question :)
OK for those wondering.
In the UK if someone is being discharged from hospital but still needs some input like District nurse, social worker, OT, Physiotherapist, Macmillian nurses (hospice) etc a package of care (care package) is hopefully done before the discharge and everything sorted.

does anyone know of any research that has been done, to show the importance of care packages?
