I can't seem to write any interventions and rationales for my care plan of
" impaired skin integrity r/t Herpes Zoster AEB multiple lesions and pruritis

Would you do anything to the skin? Put anything on it? Could there be a prescribed med you would administer? Then, the rationale : why would you do those things?

Do you have a good nursing diagnoses book. If not, then get one. They are awesome. After you find your diagnoses based on the defining characteristics you get during your assessment, it will list interventions and rationales that you just have to tailor to your patient and your goal.

You should probably start off with something like "Assess" the skin for signs of redness, warmth, and infection (as well as checking for any s/s of any new lesions, soars, pruritis etc.) Also, your pt might be in pain as well (lesions are painful) so I personally would try and add something about that in there as well.

I don't think you can say, "r/t herpes zoster" as that's a medical diagnosis.
