The renal units are so scared of what bundling means to their bottom line that they have slashed the budgets.
In my unit we have to lay off the receptionist, cut the Social worker and dietitions hours and finally this week we are told to lay off one full time RN.
We have no supervisor and no ACT-they are not needed anymore!
We have 98 pts, run 4 shift of patients
2 fulltime RN's
3 30 hour RN's
8.5 DT's
What are the staffing like in your unit and how many pts do you have??

Our unit has 76 patients
Staff:
2 full time rns
7 Full time dt
1 part time dietician
1 part time social worker
1 16 hour/week secretary
It's been like this for 3 years. We run 5 shifts.
How many pts per RN do you run?

Whew!!! I work for a large nephrology practice. I work in 3 FMC HDU's. No lay-offs at this time. The units' staffing are:
Unit 1 137 pts, 3 FT RNs, 1 FT Dietician and 1 FT SW. RNs as a rule do not run pts. Techs have 4-5 pts.
Unit 2 70 pts, 2 FT RNs, 1 PT RN
Unit 3 35 pts 2 FT RNs
However, the SW and dietitians cover more than 1 unit. Only the largest unit has its own SW and dietician, everyone else shares.
What about charge nurses?
Who does the CIA's RIA"s?
There are no dedicated charge nurses.

I am seeing the CMs working the floor 3-4 days a wk, kick out the CCHTs and RNs as fast as you can just to meet TAP. Despite that one unit was still over by 6hrs. How can that be? That CM was there 7days trying to work the floor and managerial stuff. I don't see the CMs lasting too long. I see a disaster in the making. Wait til there is only 1 RN, 2CCHTs for 15pt (5:1) and all coming off, wanting off immediatley.. one bleeding out, another cramping, another with low bp standing, etc... no sec to answer calls of when is momma ready?... don't forget post assessment and fill out adverse advents/proton note for anything "not normal" Dang....and get off the clock ASAP! all to satisfy bottom dollar??? while we jeopardize lives d/t not enough hands!!!! or is it just my region?? And of course knowing the company will terminate you if you did not do it all despite not having the opportunity to do it. Gotta triage as to what is the most crucial event at that moment...aaarrrggghhhh

FWIW, I just got hired and I don't think I was the only one. Orientation classes begin March 1st.
