I’m writing this because I searched high and low for the answer and couldn’t find it . I received disciplinary action for a med error . I stupidly gave bed 1 bed 2 medication and he end up having to go to the hospital but he was already pretty messed up but the wrong medication didn’t help him . I take full responsibility for my actions but the place I was working at had numerous ij s but it was walking distance from my house and I was a new nurse . But I could go all day about the factors but I’m the one that identified the pt by saying Hi jake and sense he said hi back I assumed it was him . But I found another job worked there 5 years and not even a write up . I have the disciplinary action on my license which was probation , fine and ceus. Which I’m grateful for . I have now completed the RN program and the disciplinary action is not on my new license . I talked to a recruiter at my state biggest health system and she said I don’t even have to mention Lpn disciplinary if I’m not asked because it is not on my license . I was hired at my first interview the day after no problem . I would put out there that it is post covid and the hospitals are hurting for nurses . Keep your head up and protect your license like it’s your baby because I know if it wasn’t for the need for nurses they would rather have someone with a clean history . And also if you do go for RN apply to your dream job right away they are more incline to hire you as a nurse resident .

However, what does your state's BON say about this? In some states, mine, for example, they want to know if you've ever had any licensure discipline, not just nursing. I guess it depends on the state.

I'm curious now. How did you answer the board's question on disciplinary action when you applied for the NCLEX?

Hey I have my Lpn license and RN license in the same state . I answered yes to having disciplinary action and submitted proof that my license is unencumbered and an explanation . And they issued my license I can log in and both are attached to my profile
