Did you? You know whether or not there was another person standing there with you when you performed a waste and whether or not said person entered their info as a witness. This is not complex. Either you did or you didn't. Whatever that answer is, check it against the policy. 

I guess if you enter in someone's information or skirt around the system.  Most facilities have a camera and you could easily see if someone was standing next to you.

Just guessing but it seems like the poster would be asking the question because they don't think they did what they are being accused of.

Sure they know if another person was there with them; they don't recall being able to waste without that being the case. But now someone has accused them of doing just that, so they are asking whether such a thing is even possible.

 

In my experience with Pyxis, if the medication was programmed to require a witness, it was not possible to complete the waste without one.  If the waste was required by unit policy, rather than the pharmacy, it is possible that the system would allow the waste without a witness.

Either way, the pharmacy should be able to run a report detailing the transaction.
