I often wonder this because where I am at, I am often wonder if the state is not strict enough with the survey.
Our MAR/TARs are computerized. You "click" off the box to "show" it was given and it puts your initials in the box. I am able to run a report that shows any holes in that MAR/TAR and I ALWAYS ALWAYS ALWAYS run that report to make sure I didn't miss clicking one of the boxes. I cannot tell you how many times i KNOW 100% that each and every box was clicked and had my initials in the box only to go back to work a fe days later and see 'holes" in the MAR/TAR. All it takes is one little computer hiccough and you're initials show but then the back up happens and its not there! GRRRR.

Personally, I like the QIS survey a lot better than the old traditional. My last 3 surveys were QIS. Most of their questions seem to go to the Nurse Managers who know the residents and what is going on with them the best. the length started out at 5 days and they have honed it down to about 3 1/2 now.
