Are there specific aoc's that are required to be done in order?
Also, for example resp assessmt, do we only chart normal or abnormal breath sound or do we go into wheezing on expiration etc?
Im in the process of making my grid with the assessments and managements. I have also included vs order to chart.
For WIIGS, is the S for skin turgor and if so doesnt that go under Fluid Mgmt with the 20min checks and not the initial intro etc?

I would like to comment on yout "L" for the Look.... a Round belly is suspected as distended...but is it? Is a Baby Belly round yes....is it distended...no
A beer belly is round....is it distended...no not usually. Please be careful when charting the "L" Look....Distension cannot be proven with looking it can only be proven/disproven with palpation. So for the look chart contour ( round or flat) and then when you palpate address distension and tenderness or distension and rigidity.
