I graduated from a nursing school in NV and planning to take NCLEX in CA. I took all my prereqs at community colleges in CA. Since all the transcripts from the schools I attended must be sent in to the board, does anyone know if I have to attach the transcript request form (on the state board's website) with each transcript? Or does that form only needs to be attached to the transcript from my nursing school?
I'm just confused because the form asked for degree and graduation date...my transcripts from the community colleges only has prereqs so I don't know if the form applies to them. I've been waiting 3 months for my test date already and really don't want to delay my process any longer. If anyone has gone through this process before, please let me know. Thanks

I am from NC and took my test in CA, I also took my pre reqs at a college other than the one I graduated from. When I talked to my registrars office they told me once you get a degree/graduate the transcripts are combined. With that being said, I only turned in one transcript release form and that was to the school I graduated from and everything went well, well the second time because the first time after paying my money my school forgot to send out my transcript.
Hi Gem0607, thanks for answering my question. I have mailed transcripts from the schools I took my prereqs from without the release form; hopefully the board will accept them. The website says they will only accept transcripts that are mailed by the schools...I actually mailed these out myself so don't know if I should request for transcripts again. Sigh...I'm so tired of this whole application process, it's actually more complicated than the test itself =(
