Ancc yellow book

So I purchase the yellow book for the Ancc exam. The very first question asks about what you find on the assessment of a 6 month old. The book gives the answer b as the correct answer, but the real answer should be c. Then reading the rationale it evens sounds like c should be the answer. Kind of frustrated, don't want the book to lead me astray! Thanks

what is the complete question and the available choices for answers? I don't have the book.

What is the "yellow book"? And which ANCC exam is this for? I have the 2-volume set that they publish and it is called the Family Nurse Practitioner Review Manual, but the books are gray, not yellow.
