Hello Everyone!
I am working on a planned change paper with an idea to create a "Change of Shift Report Form" that would be used by all nurses within a facility.
**Please respond with pros and cons and your personal opinion of the usefullness of this form in a real clinical setting.
*Using a complete report form would enable every nurse to gather important info on their patients. The nurse could fill out a Change of Shift Report Form and then report off using the same format for each patient.
*I believe this would allow for more complete and precise reports to be given and decrease the exclusion errors that often occur as a result of 'forgetfullness' at the end of the shift. Also, this form could be passed on to the new nurse, serving as a hard copy of pertinent info that could be used throughout the shift.
*What do you think?

Pros: this type of sheet would be useful for continutiy of care
Cons: the majority of my shifts I would not have time to fill out the form. and I'm sure if my shift was crazy I would be just as likely to forget something as I do in report.

My facility has a "daily report sheet". It is actually more of a booklet because there are two pts. on each page. Throughout the day the secretary write all new orders on it as well as the MAR's and the tech's and nurses all write the basic of the clients behavior and any problems or complaints that need to be passed on to the next shift. I works great at our facility to keep everything in one place and anyone who missed report can easily catch up. I work on a psych floor though so it might be different for a medical floor.

The nurse could fill out a Change of Shift Report Form and then report off using the same format for each patient.
My facility does this already. Each floor has their own.

We started this in 1991 when we went to 12h shifts. You filled it out and photocopied one for each member of the oncoming shift plus one for the Charge Nurse. All were on the same sheet of music so to say.

Our forms are printed up on night shift ready for the day with the pt. name, age, diagnosis, MD, and legal status(ie. commitment, has gaurdian), and V\S from the 0600 rounds. Throughout the day we add new orders, and bx comments and happenings.
