Hello Everyone! I am working on a planned change paper with an idea to create a "Change of Shift Report Form" that would be used by all nurses within a facility. **Please respond with pros and cons and your personal opinion of the usefullness of this form in a real clinical setting. *Using a complete report form would enable every nurse to gather important info on their patients. The nurse could fill out a Change of Shift Report Form and then report off using the same format for each patient. *I believe this would allow for more complete and precise reports to be given and decrease the exclusion errors that often occur as a result of 'forgetfullness' at the end of the shift. Also, this form could be passed on to the new nurse, serving as a hard copy of pertinent info that could be used throughout the shift. *What do you think? **Feedback may be used in my paper, and you, the source, would remain anonymous.
