Epocrates says: Avoid combo: combo may incr. amiodarone levels, risk of QT prolongation, cardiac arrhythmias (hepatic metab. inhibited; additive effects, some azole antifungals may prolong QT)
Medscape says: Amiodarone and Fluconazole both increase QTc interval. Never use combination. Fluconazole will increase the level or effect of Amiodarone by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Significant interation possible, monitor closely.
thanks, i was able to find this info online. what i'm trying to find out is if this is common knowledge to the majority or if nurses see these given together without reservation.
Sorry just playing with my itouch... got Epocrates, Medscape and Davis on it.
no problem- had you ever heard of it before looking it up just now?

I've "always" known not to give them together unless there is absolutely no other option.
with amiodarone having such a long half life, what is an appropriate time to wait between stopping amiodarone, starting diflucan and restarting amiodarone?

The half life of amiodarone is highly variable, but it's usually listed in terms of days or weeks rather than hours, so holding the amio while you infuse the diflucan wouldn't really be of any benefit.
Nope, would have looked it up because I do know about Diflucan being a trouble maker.
