I kinda work as a jack of all trades in this place. Extern, CNA, unit secretary. Well right now i'm dong 4 hours as the US here (kind of a slow day) and we got a new admission. So i take the "overbed sheet" (the paper that slides into the pocket above the pt.'s bed with the pt.'s name, doctor's name, and the pt's allergies). Well i got the pt.'s name, and the allergies on there, but i didn't finish putting the doctor's name on it due to 3 phone calls at once. So i set the paper aside.
Well while i was on the phone, one of the CAs picked up the paper and hung it upin the pt.'s room. I thought "Ok after i get x-ray squared away here, i'll go get the paper and fix the name."
Too late. The doctor came around the corner, laughing and told me that i should write slower next time and finish the whole name.
Well the first part of his name begins with C-l-i-m. The L was too short, the I wasn't dotted, and the L and I were joined together, looking like a U. It look like an entirely different word. It could have been worse if the pt. or the family pointed this out, but the doctor (who's still over there snickering :imbar ) was embarrassing enough. Another crawl-in-a-hole-and-die moment.

Nothing like spreading some smiles to the co-workers and docs on a weekend shift at work. :chuckle

Right out of nursing school, I was charting about a resident that had cellulitis and had drainage. I didn't know that it was called purulent drainage. I knew that it was pus, coming from this wound, so I charted that the patient had pussy drainage. Not realizing I was pronouncing it one way, and in the chart it looked like something totally different. Luckily I was working with my sister who is also a nurse and she caught the mistake, but that pesky little rule of only drawing a line through the word, still made it readable to others. But that's okay, because one day my sister was charting and she had her head somewhere else and charted that the patients IV was patent to DD, with clear yellow urine. So we all been there lol
And also as a Care plan coordinator, I had careplanned for heel protectors to be on at all times while in bed, for a patient that was a bilateral lower amputee. Not so lucky this time, it was caught by the state surveyor lol :uhoh21:
