I do. Is there a separate section for allergies?

Anything with a preexisting med would qualify as far as I'm concerned.

no separate section and do you count your ashtmatics that come in daily as DAILY MEDS GIVEN i always considered them as prn meds and wasn't counting them.

Mine don't come in daily. I'm at the HS level and they self carry. I only document if they come in with symptoms, or need a Neb.

In Pennsylvania reportable conditions are: Life threatening allergy, life threatening food allergy, asthma, bleeding disorder, ADD, ADHD, torrette's, diabetes, arthritis/rheumatic, cardiovascular conditions(lumped together), cerebral palsy, cyctic fibrosis, epilepsy/seizure, sickle cells, and spina bifida. These are counted even the nurse never sees the kid. Meds are counted as Analgesic, antibiotic, anticonvulsant, versed, diastat, antihistamine, epi, anti-inflammatory, asthmatic, diabetic oral, insulin, glucose, glucagon, GI, GI enzymes, ADD/ADHD, other psychotropics, and other-these count per administration.

Hope that helps!
