I work in a fairly small hospital Birth Center. Our patients are usually compliant with docs orders etc.. But twice in the last week, I had patients that were absolutely refusing treatment. The first was preterm labor (34+ weeks) throwing up, contracting and couldn't keep her PO terbutaline down. Got orders for a liter of IV fluids, zofran and SQ terb. She refused, up one side and down the other - wanted to leave, yada yada yada. Her boyfriend convinced her after about an hour to take the meds and IV, but she told me I had to have someone come and hold her arm down because she "gave a doctor a black eye the last time I had an IV".

Last night I get a 17y/o married but separated with a boyfriend, G2P0 (37 week demise a year ago), cocaine + with lady partsl bleeding at 18 6/7 weeks. She had left ER AMA about 12 hours earlier and came back for treatment when she could get someone to come in with her. She spent the whole time with the room phone in one ear and her cell phone in the other, screaming and yelling, cussing up a storm and left AMA about 4 hours after she got there.

What is with people?!?!?!?

Our other patients, should be 'normal' have been rude, snippy, refusing to allow us to do regular post-partum tummy checks, etc...

We have had that kind of fun recently with a few "Bradley patients." If they think that the medical community is the root of all evil then why do they come to us?

That's too bad, I did Bradley with my babes and it was great. People who are rude ruin it for everyone else.

Maybe they wanted a homebirth or birth center and their insurance wouldn't cover it. I was fortunate that the only hospital covered by my insurance was very progressive.

Had a 22 wk gestation witth cramping and bleeding 1 week beloow the cutoff for our ld triage refused to go down to ER because an ex boyfriend worked in housekeeping she left AMA and drove 45 minutes to another ER in 5 oclock traffic which likely doubled her ETA. i have no idea what goes on in these pts minds!

i once had a pt come in for c/o pih symptoms. she refused to have labs done and refused the iv on the advice of her friend who then convinced her to get "just the heplock" and no fluids.....why???? she eventually left ama.

my experience w/pt who are wanting a heplock is for the iv access for their street drugs, is that what you were stating or did i miss the point? :chair:

The story about the 22wk pt who refused a visit to the ER raises some alarm bells...

It could be the ex-boyfriend who worked downstairs was/is abusive. Pt may be avoiding this individual for reasons beyond just vanity. I may be waaaay off base but I wonder...

Normally, I have very few pts who just annoy me, but we had one who took the cake the other night. 20 yo primip came in early morning 2 cm with SROM. All day, she annoyed the day shift nurses refusing VE's and to keep the monitors on. Continued through night shift. She was just plain mean. Normally we don't do continuous monitoring but she had been ruptured at least 20 hours by this time, and had two episodes of decels down to the 60's. She would take off the external monitor repeatedly, refused internal monitoring, wanted an epidural, but refused VE's. When told the monitoring was for the safety of her baby, she said "I don't care". Needless to say, she got a social work consult.
