here we go again...

i'm working on a concept map for a case scenario. no real patient to asses...just the information given by the instructor.

"patient" is an 82 year old female diagnosed with type ii diabetes mellitus and dementia. she is experiencing hyperglycemia with blood sugars running consistently over 285. she is hungry all the time and forgets that she has eaten.

i do not, however, know if the "patient" is overweight.

i want to use the diagnosis imbalanced nutrition: more than body requirements based on the fact that she is eating all the time. were this a real patient, i would assess her dietary habits, weight, glucose levels, and activity level. i feel like there is more i could look at, but at the moment i'm drawing a blank.

any help would be greatly appreciated!

sorry, should have been more clear . . . this was my point. she isn't eating more than her body requires. she needs insulin to get the sugar out of the blood and into the cells. eating more won't do that. and you do lose weight even though you are stuffing yourself.

i've told my dad that if i end up with renal failure because of dm2, i'm gonna order some large pizzas, 3 liter cokes (regular) and close the door :) they can fetch my remains in about 3 weeks, or if i stink sooner than that :up:

try these:

ineffective self health maintenance (domain 1, health promotion)

risk for unstable blood glucose (domain 2, nutrition) (yeah, i know she's already doing it, but it's still an ongoing risk)

chronic confusion (domain 5, perception/cognition)

impaired memory (same)

honest to god, i find the easiest way to identify potential nursing diagnoses is to take the nanda book, go to the title page for each domain, and read the darn list. if one seems right, i can flip to its page for defining characteristics to see if it really fits my assessment.

otoh, if everybody did that, no one would come to an to ask us! :D and then what would we do c our spare time?

I actually do that myself. Problem is, when I'm working with a case scenario and not a real patient, I don't have the opportunity to assess for further information. So I'm stuck trying to figure out what works based on the little I've got.
