Tachycardia nursing diagnosis? time sensitive!

Good Evening all,

I was told that I must come up with a nursing diagnosis and some interventions for : tachycardia

I went all over my nursing care plan book, and nanda books and could not find tachycardia!

they barelly touch it, they get into other dysrhythmias but not tachy...

this is for tomorrow early AM, can some one guide or gelp me out on this???

Ohhh...how about impaired gas exchange?! Could be tachycardic because they're not getting enough oxygen...?

I hate these types of assignments where they basically give you nothing to work with...lol.

see i have analyzed those 2 websites...

all i have in my mind is :

drug interaction?

anemia?

endocrine?

unexpected blood flow changes?

blood loss?

CNS stimulants???

but i cant come up with a diagnosis for tachy!!!

Decreased Cardiac Output r/t altered heart rate and rhythm AEB atrial dysrhythmia and ventricular rate of 160 bpm

Interventions:

Assess VS, LOC, SOB, pallor/cyanosis, weakness/fatigue, peripheral edema, dizziness

Have patient perform valsalva maneuver

Administer O2

Begin cardiac monitoring

Maintain IV access

Have resuscitation equipment available

EKG

Check for history of cardiac issues, diabetes, pulmonary diseases, thyroid issues

Ask about history of alcohol/drug use

Anticipate orders for medications and labs to check electrolytes, h/h, thyroid panel

Expected outcome: Within one hour of interventions patient will have normal cardiac output as evidenced by HR of 60-100 bpm, normal sinus rhythm on EKG

:D it makes sense to me now!!

cardiac output decreased....

could be related to

low demand of myocardial o2

could be loss of blood

... drug interaction/reaction/AE

OK...!

it all makes sense

see? yall triggered my brain for me!... I guess I was too tired

NANDA for me: readiness for enhanced sleep!!
