How to use nursing process to identify diagnosis?

I'm a 1st semester NS. This week I has a pt with a hip prosthesis dislocation, and instead of having surgery pt had a closed reduction (just had the joint popped back into place). Pt was in her 70's & had arthritis. Other than that, had high blood pressure. I'm trying to identify a nursing diagnosis and goal that would be appropriate for this pt. All other vital signs were in normal range.

What about labs, the rest of your assessment, patient medications? What about psycho social and emotional status? . What about when your patients goes home and any patient teaching your patient may need?

If you're in the first semester, your instructor might want you to keep it simple. For example, Impaired Mobility related to hip prosthesis dislocation, etc etc.

Pain, discharge planning, alteration in health care....

I am a huge one on the discharge process. What is this patient going to do when she gets home? VNA? PT? OT?

For this pt pain would be a huge issue, there is more than likely chronic pain from her OA, add in the acute pain r/t the dislocation of the prosthetic hip joint. Depending on her level of pain this is going to possibly make her not want to move around much.
