Help with first nursing exam questions!

hi there lovely nursing folk :nurse:

i have my very first nursing exam in a weeks time and have been studying last years exam and theres a few multichoice questions that just don't seem clear to me.... any ideas or feedback would be much appreciated!

during the nursing assessment, mrs jones becomes very tired, but there are still questions to be asked to complete the care plan. which of the following actions would be the most appropriate in this situation?

a) ask mrs jones to wake up and try to answer the questions

b) ask mrs jones' husband to answer your questions

c) wait until the next day to obtain the answers to your questions

d) ask mrs jones if you can ask her daughter to answer the questions

hmmm my first instinct would be to wait until the next day but then what if it can't wait...and who would be the next secondary source of info... husband or daughter?

a) culture guides behaviour into acceptable ways for people in a specific group

b) culture is not affected by a group's social and physical environment

c) culture practices and beliefs are constantly evolving

d) there are differences both within cultures and among cultures

with c, the main practices stay the same(ish) i believe and with b, the group's social and physical environment definitely affects culture. so wouldn't this be both a and d?

which of the following is not a recommended technique to help improve listening skills?

a) always maintain eye contact with the patient in a face-to-face position - i'd say yes

b) use appropriate facial expressions and body gestures to indicate you are paying attention - i'd say yes

c) think before responding to the patient, even if created a lull in the conversation - this is the only maybe? but isn't it good to think before responding?? seems the only one it could be...

d) listen for themes in the patient's conversation - i'd say yes

which of the following techniques would a nurse employ when using listening skills appropriately?

a) the nurse would try to avoid body gestures

b) the nurse would not allow conversation to lapse into periods of silence

c) the nurse would listen to the theme's in the patient's comments

d) the nurse would stand close to the patient when communicating with him/her and maintain eye contact

ummmm wouldn't this be both c and b??

which of the following statements accurately describe the effects of foods on bowel elimination?

a) people who lack the enzyme lactase may experience diarrhea or flatulence when consuming starchy foods

b) a personw ho is constipated should eat eggs and pasta to relieve the condition

c) fruits and vegetables have a laxative eggect

d) alcohol and coffee tend to have a constipating effect

i would go with c?

personally, i think a few of these questions are a bit ambiguous. your help would be much appreciated! :p

if i were you i'd review communication and test taking strategies. usually questions that are phrased with words like "always" or "never" aren't the right choices. also, when there are two choices that are almost exactly alike (like #1 with ask husband then ask daughter) you would rule them both out because they are saying the same thing. as for the communication aspect, constant eye contact in a face-to-face position can be taken as threatening or intimidating in some cultures. also, silence can be therapeutic; it's not always a bad thing.

On the first question, asking the patient for permission to ask her daughter would be the correct answer. If the patient was fine answering the questions on her own until getting tired, you would definitely want permission from the patient to get further info from a family member. I would only question the husband without the patient's consent if the patient was unable to answer and the husband had the medical or durable power of attorney. #3, I wouldn't always maintain eye contact with a patient to show I'm listening. This can be intimidating for some people.
