When giving several meds via g-tube and two 60cc water filled syringes, I alternate meds with water flush. How would you document this? I document: "Meds given via g-tube alternating with 120cc of water flush.

I simply document "G-tube" as the medication route and then record the flush volume as enteral water on the I&O flow sheet.

Our eMAR has a spot for documenting water given pre and post med. I bypass it if I am giving a water bolus at the same time, because that is the water I am using for the meds. Can you add it to your MAR, or do you have to write a progress note?

To me it reads as if you are giving 120 ml flush each time.

If you are doing a narative, step by step.. because ckh23 is correct, it looks like a 120 flush for ea med..

If you want to be clear with the volume of water given I would write it something like this.. Meds given via g-tube mixed with 120cc of water. The word alternating implies that you gave the patient 120cc of water more than once.

I might document that like this: "Meds given via g-tube with total flush volume 120cc water, medication alternated with small volume water flush" or like this, "Meds given via g-tube alternating with a small volume water flush, total water flush 120cc."

I'm not sure why you have to document so much detail as it is just meds flushed with water, but how about:

Medications given via g-tube with water flushes in between each medication. Total of 120 ml water given.

(Always use ml as cc is not an approved abbreviation anymore.)

Yes, it sounds better that way. I would add the total at the end. "Alternating with 60ml water flush for a total of 120ml."
