CPNE Physical....no form?

The only thing I have found for the CPNE physical that is required is the immunization record that needs to be filled out, signed by self and MD, a form explaining what needs to be done, but no form for the doc to fill out relating to physical itself? Missing something? I am sure it is possible, I did this after having a rant about the Transitions practice exam so who knows.

That is the physical you are looking for. You and a MD sign and thats it!!

I was looking for a head to toe assessment but there is not one. The health status form aka physical is the MD signing that you are up to date on injections and healthy.

Does he need to do a dictation or something? No place for vitals, etc?! Ugh....

Nope. Isn't there still a line that says something like "I certify that this individual is in satisfactory physical conditioning," etc. blah blah blah? That sums it up.

Nope, they are just verifying you are up to date on your shots and you are in good enough physical condition to do your clinicals... my doctor seriously scribbled his initials and stamped it with his stamp and they were happy w/that.

My MD signed in the wrong place! He was like "Ehhh whatever!"
