reference request

Hey guys, I got an application from the career fair (about the only place that is hiring). Anyway, the app has 3 sheets for reference requests. They all have work, education, and personal ref. sections. I'm not sure how much of it to fill out. There is a TO: at the top and a box by each work, edu., and pers., but none checked. I assume you fill out the top part with your name, SS, ect., but how do they know who is supposed to fill out the other sections unless I right it in, but there is no spaces for phone numbers. I was thinking of filling out 1 of the sheets as I don't think I can come up with 3 personal references unless I can use the same ones as work references. Any advice or should I call the lady I talked to today and ask her about it? (she said to fill it out and then call to set up an interview and bring it with me).
