Kind of long post about career decision

Hello,

I was wondering if anyone could give me some insight into my problem. I have just completed my nursing degree and while I was in school I was able to work as a student at a hospital, so I was known by the nurse recruiter for that hospital. I sent my application to him and he sent it to a transitional care unit as per my request. I received an offer for the job, but I turned it down. The manager of that unit called the nurse recruiter informing him of my decision and the recruiter in turn called me to find out what my plans are. I told him I wanted to wait until I had finished writing my exam, and I told him I would update him near that time. When I turned down the position at the TCU, I was told the offer was mine to take if they still had any positions left at the time I decide I would like to work. My problem is that I think I my decision may have been troublesome to the nurse recruiter as it is his job to recruit nurses for vacancies and here I am telling him I'm ready to work when I realize I'm not and I turn down the position still leaving a vacancy. I now almost feel obligated to take that position when I've finished my exam, but I don't think I want that position, but I don't want to ruin his opinion of me in the case that I do want a job in the future as this nurse recruiter is in charge of the majority of hospitals and other agencies in my area. I don't know if I should take the original job offer if it is available when I am ready or take a job in any of the agencies of which the recruiter is in charge to maintain good relations with him, or if I should take a position in a hospital outside of the health authority in order to get a job with which I may be happier. I think what's making my decision difficult is that I really don't want a med-surg type job and I'm having a hard time confessing to him that I want something slower paced like long term care so that he will send my resume to those areas. It's all ridiculous I know, but any insight or opinion at all would be helpful.
