How to I bill medicare?

I have been a LPN in the state of NY for 8 years, working in a nursing home and then a local hospital. I recently quit my job to start doing homecare. I have applied for my EIN number and I have a NPI number. Before I could get any clients my grandfather went into the hospital with pneumonia. The doctor recommends nursing home care and it has been approved by medicare. My grandmother asked me to do it and it would be very convenient for me in location. They live in front of me. My question though is how do you bill MEDICARE? The only thing I find online is for MEDICAID. Do I need a medicaid number? I am so confused.

it seems like a lot to do, especially if it's for just one person. What if you went to a licensed agency, and contracted to provide services through them?
